Investigators say pilot error led to a deadly plane crash in Monroe last year.
The crash on March 29, 2011 killed pilot Rick Howell and passengers Nathan Brahier and Jeremy Tate.
The National Transportation and Safety Board determined that Howell told an instructor that he was going to do a high-speed pass over the runway.
Witness and radar data showed that he came in too fast and his propeller struck the runway, which caused a loss in thrust. The plane went down and caught fire. The report says the landing gear and flaps were all retracted.
The plane ended up in Munson park, but no one on the ground was injured.
Toxicology tests showed the presence of chemicals consistent with cold or pain medication in Howell's system, but they could not determine the levels of the chemicals or what, if any, role they played in the accident.
